Effectiveness of Long-Acting Antiretroviral Therapy and the U=U Strategy in Reducing HIV Transmission: A Contextual Study in Cuba

Abstract
La estrategia Indetectable=Intransmisible (I=I) redefine la prevención del VIH. El objetivo fue evaluar la efectividad del tratamiento antirretroviral de larga duración (TLD) en Cuba. Estudio observacional, retrospectivo, multicéntrico en 500 usuarios (2024-2025), edad promedio 38,6 años (67% hombres, 33% mujeres). Resultados: 95% logró supresión viral (<50 copias/ml); reducción del 90% en transmisión; 88% alcanzó adherencia ≥95%; efectos adversos en 12%. La simplicidad del TLD favoreció la continuidad. Conclusiones: Se confirma la eficacia del TLD y la validez de I=I en contextos caribeños con recursos limitados, reforzando su aplicabilidad global.
